0:00Hi, good morning Collins. My name is

0:02Mahera. Let's start the warm-up session.

0:06Why did you choose to become a nurse?

0:08>> There are several reasons why I chose

0:09nursing. First of all, many of my

0:12relatives are nurses and their stories

0:14inspired me a lot. Besides, nursing is a

0:17profession with an ocean of

0:19opportunities both native and abroad.

0:21Above all, this is a service-oriented

0:24profession in which I can serve

0:26humanity. Hence, I chose nursing as my

0:28career.

0:29>> What is your career goal after reaching

0:31your dream destination?

0:33>> Right now, I do not have any clear

0:35management goals. I am very interested

0:37in learning about management and

0:39leadership and would be interested in

0:41any committees that I could participate

0:43in and learn. I have lots of ideas and

0:46would love to be in a brainstorming

0:48group and I am very interested in the

0:50management dynamics of the organization.

0:52I feel that I need to join leadership

0:54groups to learn and meet new people.

0:56>> Describe your typical working day.

0:59>> My day starts with prayer. After

1:01completing my daily routines, I reach my

1:03work station by 7:30. Before taking the

1:06handover, I complete my inventories and

1:09update the crash cart medication and

1:11dressing trolley. After receiving the

1:13handover, I interact with my patients to

1:15identify their health needs. Based on my

1:18assessment plan, a nursing care plan and

1:21provide quality care for my patients.

1:23Afterward, I assist the physician in his

1:26work and carry out necessary clinical

1:28orders. Meanwhile, I find some time to

1:30interact with my colleagues and maintain

1:33a good rapport with them. My duty ends

1:35by 3:30 after I give a handover to the

1:37next scheduled staff members.

1:39>> Where do you see yourself in 5 years?

1:42>> In 5 years, I envision myself as a

1:44seasoned nurse working in a challenging

1:47and rewarding role. I aspire to deepen

1:50my clinical expertise and leadership

1:52skills and to become a valuable asset to

1:54my health care team. I hope to be

1:56involved in mentoring and educating new

1:59nurses and to contribute to improving

2:01patient outcomes through evidence-based

2:03practice. Ultimately, I strive to make a

2:06positive impact on the lives of my

2:08patients and their families.

2:10>> What are the pieces of advice for the

2:12upcoming nurses?

2:13>> As a nurse, I can give a bunch of advice

2:16to upcoming nurses. Firstly, I advise

2:18them to love their profession. If anyone

2:21loves their profession, they can carry

2:23out their responsibilities in an

2:25efficient manner without complaints.

2:27Apart from that, I advise them to be

2:29dedicated to their profession and

2:31maintain professional standards. These

2:33are essential because a nurse should

2:35abide by nursing ethics on any grounds.

2:38These are a few pieces of advice that I

2:40would like to give to future nurses.

5:42>> Your preparation time is over. You can

5:45now start your role play.

5:48>> Good morning. Welcome to the Community

5:50Health Center. My name is Henzy

5:52McCurley, and I am one of the nurses on

5:54duty today. May I please have your name?

5:57>> Hello, Nurse Henzy. My name is Mahira.

6:00>> It is very nice to meet you, Mahira. How

6:02can I help you today? Could you tell me

6:04what brings you to the health center?

6:06>> Oh, Nurse, I am feeling terrible. I have

6:08been throwing up and having really bad

6:11watery diarrhea, along with awful

6:13stomach cramps. It all started so

6:16suddenly. I really need some medication

6:18to stop this right away.

6:20>> I am so sorry to hear that you are

6:21feeling so unwell, Mahira. Dealing with

6:24stomach troubles can be very

6:26uncomfortable and exhausting, so I

6:28completely understand why you are

6:30seeking help. To help me get a clear

6:32picture of what is going on and how we

6:34can best support you, I would like to

6:36ask you some questions about your

6:37symptoms first, and then we can discuss

6:40how to manage this together. To begin

6:42with, could you describe how these

6:44symptoms started and what you have been

6:45experiencing over the last day or two?

6:48>> It started about 24 to 48 hours ago out

6:51of nowhere. First, I felt extremely

6:54nauseous, and then the sudden vomiting

6:56began. Shortly after that, the watery

6:59diarrhea started, and my stomach has

7:02been cramping up ever since. I feel so

7:05weak and tired from going to the

7:06bathroom so many times.

7:08>> That sounds really draining, Mahira.

7:10Thank you for sharing that. Along with

7:12the vomiting and diarrhea, have you

7:14noticed any signs that might suggest you

7:17are getting dehydrated, such as feeling

7:19dizzy when you stand up, or noticing

7:21that your urine is much darker than

7:23usual?

7:24>> Now that you mention it, I did feel a

7:26bit light-headed when I got out of bed

7:27this morning. I haven't really checked

7:30the color of my urine, but I know I

7:32haven't needed to go to the toilet to

7:33pass urine very much today because

7:35everything seems to be coming out as

7:37diarrhea.

7:38>> I appreciate you letting me know about

7:40the light-headedness, as that can

7:42certainly happen when your body loses

7:44fluid. Besides the vomiting, diarrhea,

7:47and cramping, are you experiencing any

7:49other symptoms?

7:51>> I don't think I have a high fever,

7:52though I felt a bit feverish last night.

7:55There's no blood in my stool, thank

7:57goodness, and the stomach pain comes in

8:00waves with the cramps, rather than being

8:02constant and sharp.

8:04>> All right. Mahira, what could be the

8:06possible cause for this?

8:08>> As for causes, I haven't traveled

8:10anywhere recently, but I did eat some

8:12takeaway food 2 nights ago that tasted a

8:14bit off. I wonder if that caused this.

8:17>> That is very helpful context, Mahira.

8:20Eating something that was slightly off

8:22can certainly trigger an upset stomach.

8:24Now that I have a better understanding

8:26of your situation, I would like to

8:28outline some effective ways to manage

8:30gastroenteritis at home, so you can

8:32start recovering comfortably. First, the

8:35most crucial part of your recovery is

8:37hydration. Because your body loses

8:40essential fluids and salts through

8:42vomiting and diarrhea, you need to

8:44replace them by drinking oral

8:46rehydration solutions or water. It is

8:49best to sip small amounts frequently

8:51throughout the day, rather than drinking

8:53large quantities at once, which might

8:55upset your stomach again. Next, getting

8:58plenty of rest will give your body the

9:00energy it needs to recover. When you

9:02feel ready to eat again, you should

9:04gradually reintroduce bland foods as

9:06tolerated, such as plain toast or white

9:09rice. While avoiding rich, greasy, or

9:12spicy foods for a while. Finally,

9:15practicing strict hand hygiene by

9:17washing your hands thoroughly with soap

9:19and water, especially after using the

9:21bathroom and before handling food, is

9:24essential to prevent spreading any germs

9:26to others. Mahira, regarding

9:28over-the-counter support, you can

9:30purchase oral rehydration salts from the

9:32pharmacy to help restore your

9:34electrolyte balance. You might also

9:36consider antidiarrheal medications like

9:39loperamide for short-term relief if you

9:41need to manage the diarrhea, though

9:43keeping hydrated remains the main

9:45priority.

9:46>> Thank you for explaining all of that,

9:48nurse. But can't you just prescribe me a

9:50strong antibiotic or something stronger?

9:53I really want this to stop immediately

9:55so I can get back to work.

9:57>> I completely understand your desire to

9:59get back on your feet quickly, Mahira,

10:02and it is completely natural to want a

10:04fast solution when you are feeling this

10:06miserable. However, I must gently advise

10:09against prescribing antibiotics in this

10:12situation. Most cases of gastroenteritis

10:15are caused by viral infections, and

10:17antibiotics are strictly designed to

10:19treat bacterial infections, meaning they

10:22are ineffective against viruses. Taking

10:24unnecessary antibiotics can actually do

10:27more harm than good, as they can cause

10:30unwanted side effects like further

10:31stomach upset or thrush, and they

10:34contribute to antibiotic resistance.

10:36Furthermore, as a nurse, issuing

10:38prescription medications for viral

10:40gastroenteritis is not within my scope

10:43of practice. Mahira, please be reassured

10:46that gastroenteritis is usually a

10:48self-limiting condition. This means your

10:51body's immune system will naturally

10:53clear the illness, and symptoms

10:55typically resolve on their own within a

10:57few days to a week without causing any

10:59long-term complications. How are you

11:02feeling about these management steps?

11:04And do you have any specific concerns or

11:06questions?

11:07>> That makes sense about the antibiotics,

11:09even if I was hoping for a quick fix. I

11:12guess I'm just worried about how to tell

11:14if I'm getting worse or when I should

11:16actually be concerned.

11:18>> That is a very valid concern, Mahira,

11:20and it is important to know what warning

11:22signs to watch out for. Now, I would

11:24like to explain when it is necessary to

11:26see a general practitioner or seek

11:29urgent medical attention. You should

11:31consult a doctor or go to urgent care if

11:33you experience persistent vomiting that

11:35prevents you from keeping any fluids

11:37down for more than 24 hours, or if your

11:40overall symptoms last longer than a few

11:42days without any improvement. You must

11:45also seek immediate care if you notice

11:47severe blood in your stool, develop a

11:49high fever, or experience pronounced

11:52signs of severe dehydration, such as

11:55persistent dizziness, confusion, extreme

11:58thirst, or passing little to no urine.

12:01If any of these red flag symptoms occur,

12:03please do not hesitate to get evaluated

12:06right away.

12:07>> All right, Nurse Henzie. I will buy some

12:09rehydration salts, rest up, sip fluids,

12:12and monitor how I feel over the next

12:14couple of days. If things don't improve,

12:17or if I notice those warning signs, I'll

12:20see a doctor immediately. Thank you so

12:22much for your guidance today.

12:24>> You are very welcome, Tahira. Take good

12:26care of yourself, get plenty of rest,

12:29and I hope you feel much better very

12:30soon.
